Work Authorization & Credit Card Payment Authorization Form

Sign and complete this form to authorize HoodAircraft LLC, dba SkyBlue Aircraft Maintenance the following:

• By signing this form, you give us permission to debit your account for the amount indicated on or after the indicated date. This is permission for single transaction or/and future transaction with additional 5% credit card fees, and does not provide authorization for any additional unrelated debits or credits to your account.
• I hereby authorize maintenance and repair of the above aircraft. I agree that HoodAircraft LLC, dba SkyBlue Aircraft Maintenance is not responsible for loss of articles left in aircraft caused by fire, theft or any other cause beyond our control or for delays caused by the unavailability of parts or shipping delays. I also grant permission to HoodAircraft LLC, dba SkyBlue Aircraft Maintenance employees to operate above stated aircraft for the purpose of testing and or inspection. I understand and agree to secure payment for repairs thereto, an expressed mechanic’s lien on the above aircraft is acknowledged and further agree to pay reasonable attorney’s fees and court costs in the event the legal action becomes necessary to enforce this contract. If payment is not received after 30 days interest will accrue at rate 1.5% per month.
• Direct Pay Authorization I hereby authorize payment to be made to HoodAircraft LLC, dba SkyBlue Aircraft Maintenance for any repairs made to the aircraft.
